Requirements for getting a passport for a 16 and 17 year old.

Application Form: Fill out Form DS-11 online. This form is also available at passport agencies and acceptance facilities.

  1. Proof of Citizenship: A birth certificate, previously issued, undamaged U.S. passport, or a Certificate of Naturalization.

  2. Proof of Relationship: Birth Certificate.

  3. Identification: If the minor has any I.D., please present it.

  4. Parental Consent: One parent must be present to apply for the passport with the child. Parent must present primary I.D. (Passport or Driver’s License)

  5. Passport Photo: Two passport photo meeting the requirements. Must be submitted with the application.

  6. Fees: The application fee and any additional services fees must be paid.

  7. Travel Plans: It's helpful to have travel plans or a travel date when applying for a passport.
