Signed passport, valid for at least six more months upon your arrival date in Brazil & with 2 blank visa pages
Foreign passport holders only - A copy of the U.S. Alien Resident Card and/or valid visa or document showing legal status in the U.S.
One completely filled out online Application, printed, signed, and dated. Link: https://scedv.serpro.gov.br (Click on "Visa Request", read the instructions & click on "Next" at the bottom of the page to start your application.)
* For minors (under 18 years): Both parents/guardians must sign the application *
One passport sized photo (2x2), with a white background
A confirmed round trip ticket or itinerary
Proof of address: copy of state issued photo Identification or Utility Bill with applicants name and address.
Applicable to BUSINESS VISAS ONLY: A signed letter (on company's letterhead) stating employee's name and title, contact in Brazil, job description to be conducted in Brazil, how long the employee has been working for the company, and assuming full financial responsibility for the applicant while in Brazil ; or an invitation letter from company that you’re visiting in Brazil for business.
For Minors (under 18 years) ONLY:
- Original + one copy of birth certificate
- A notarized Letter of Consent form, signed by both parents
