You can only use this form if you changed your name within one year of receiving your passport. Please make sure that you sign all the documents in your new name. If you have any identification in your new name please provide a copy and sign the same way as on your I.D

Original Court Documentation
Please provide the original court document that changes your name. For example, marriage certificate, divorce or legal name change document.
Authorization Letter
Print two authorization letters. This allows us to act as an agent on your behalf.
Travel Itinerary
Provide an International ticket, e-ticket, itinerary or hotel confirmation for those driving. If traveling on business and no itinerary is available, a letter, signed by someone at the company, on company letterhead, which indicates the dates, location, name of person traveling and purpose of travel will suffice.
Current Passport
Passport must be in good condition. It will not be accepted if damaged or mutilated. Lamination must not be peeling off of the photo page of the passport.
Government Fees
A check or money order made payable to the "U.S. Department of State" in the amount of $60.00. Temporary or starter checks will not be accepted.

Fedex Shipping
Choose a method from the following choices:
· Either include your own return FedEx prepaid airbill
· Pay $30.00 for overnight delivery Mon-Fri (for morning delivery bet. 10-11am)
· Pay $65.00 for first A.M. (usually by 8:30am) and Saturday delivery.
· Or just simply pick up at our location.
