Authorization Letter
Print two authorization letters. This allows us to act as an agent on your behalf.
Travel Itinerary
Provide a ticket, e-ticket, itinerary or hotel confirmation for those driving. If traveling on business and no itinerary is available, a letter, signed by someone at the company, on company letterhead, which indicates the dates, location, name of person traveling and purpose of travel will suffice.
Current Passport
Passport must be in good condition. It will not be accepted if damaged or mutilated. Lamination must not be peeling off of the photo page of the passport.
Government Fees
A check or money order made payable to the "U.S. Department of State" in the amount of $60.00. Temporary or starter checks will not be accepted.

Getting Your Passport
Either include your own Fedex prepaid airbill or pay $26.00 for delivery Mon-Fri (for morning delivery bet. 10-11), $55.00 for first A.M. (8:00 A.M.) and Saturday delivery. Or just simply pick up at our location.
